The Voice Intake & EHR Integration Engine (Module 2) automates clinical note-taking during doctor consultations. It transcribes the live code-switched conversation, auto-fills a medical chart, and builds a structured SOAP draft with FHIR / EHR JSON output. Every field stays a draft until a physician verifies it.

Record a consultation

Start recording

Select Start Consultation Recording, or Load Case 1 of 5 Gold Standard Cases for a scripted example.

Tag each speaker

Under Recording Speaker, select Doctor or Patient before each segment.

Watch the chart fill in

The Auto-Filled EHR Medical Chart updates as fields are recognized: chief complaint, duration, BP, pulse, temperature, and prescribed medications.

Generate the SOAP draft

Select Generate SOAP & ICD-10. Edit any field with Edit SOAP Draft. Fields you change manually are not overwritten by streaming updates.

Verify and sign off

Review the draft, then select Verify & Sign Off Record.

Export or commit

Use Preview FHIR JSON, Export FHIR JSON, Summary PDF/Text, or Commit to EHR.

The SOAP draft

  • S - Subjective: symptoms reported by the patient.
  • O - Objective: observations and vitals.
  • A - Assessment: ICD-10 code suggestion.
  • P - Plan & Prescription: treatment and stewardship notes.
The backend validates structured SOAP JSON. Free prose returns a manual-review draft and does not generate ICD-10 codes. Prose-to-SOAP/ICD-10 generation is not complete.
Physician verification is required before any FHIR EMR sync. Medication safety is not enforced end to end, so verify medication details independently.
